Minute 38 of a 45-minute call. The main topic is settled, the energy is dropping, and someone says, casually, on the way to goodbye, "oh, and can you loop in the security team before we sign off? They'll want to see the data-flow piece." You say "yep, will do." You mean it. You are also, at that exact moment, typing a note about the previous topic, watching a chat message arrive, and thinking about your next meeting, which starts in four minutes. The request was made once, verbally, mid-transition. It was never in the agenda, so it isn't in your prep notes. It came after you'd stopped taking notes, so it isn't in your meeting notes either. It exists in exactly one place: your short-term memory, which the next meeting is about to overwrite. Three weeks later the sign-off stalls because security was never looped in. Nobody remembers whose job that was — except the person who asked, who remembers precisely. Notice what didn't fail. Your diligence didn't fail; you said yes in good faith. The agenda didn't fail; the request was never on it. What failed is the capture mechanism, which was your attention, which was fully spent on the meeting itself. The requests that sink you are almost never the headline items. They're the passing ones: precise, small, and easy to say yes to while doing something else. ## Why note-taking fails under load The standard answer is "take better notes." It doesn't survive contact with a real calendar, for a structural reason: note-taking quality is inversely correlated with meeting importance. In a low-stakes meeting, you can transcribe everything — you're barely needed. In a high-stakes meeting, you're negotiating, presenting, answering questions, reading the room. The more the meeting matters, the more it demands your full attention, and the less of it is left for writing. So your worst notes systematically come from your most important meetings. The client negotiation gets "pricing — flexible?"; the routine status call gets a beautiful structured summary nobody needs. Load makes it worse. One meeting a day, and memory covers the gap between what you wrote and what was said. Five meetings a day, and each one partially overwrites the last. By Friday, Tuesday's passing request isn't inaccurate in your memory; it's absent. ## The fixes that half-work **The delegated note-taker.** Someone is assigned to take notes. Works when that person has no other role in the meeting, which is rare and expensive. In practice the note-taker is also a participant, filtering through their own attention: they capture what seemed important to them, and the aside directed specifically at you often doesn't make the cut. **The recording you never rewatch.** Recording feels like insurance, and as insurance it half-is. But retrieving one sentence from a 45-minute video means scrubbing through it — so nobody does, until something has already gone wrong. A recording preserves the meeting; it doesn't surface the commitment. And the commitment you don't see again within a day or two is a commitment you don't act on. **The end-of-meeting recap ritual.** "Let's recap action items before we go" is genuinely good practice — do it. But it catches the items everyone already knows about. The minute-38 aside, agreed and mentally filed as done-by-saying-yes, rarely resurfaces in the recap. And the meetings that overrun — often the important ones — skip the ritual entirely. All three share the same flaw: they still route capture through human attention, just someone else's or later. ## The systemic fix: capture that doesn't depend on you The fix is to build capture into the meeting itself, so it no longer depends on anything you do or notice while the meeting is happening. That means three things working together. First, a **transcript of every meeting** — both sides of the conversation, produced while the call happens, not reconstructed after. Second, **extraction**: after the call, the action items and key points are pulled out of the transcript into a short summary you actually read. The passing request stops being one sentence buried in 45 minutes and becomes a line item in front of you an hour later, while acting on it is still cheap. Third, **search**: weeks later, when the question is "did we commit to that, and in what words?", the answer is a search over your meeting history, not a memory contest. This is what Pavleur does. Don't try to recall the whole meeting — memory for "was there an ask?" is much better than memory for content. 2. **Check the artifacts you do have**: chat threads around each meeting, the meeting's shared doc, your fragmentary notes. Asks often leave a shadow — a link someone posted, a "per our call" email. 3. **For each probable ask you can't reconstruct precisely, ask directly.** The script that works: "Before it slips — you asked me for something on Tuesday's call and I want to make sure I get it exactly right. Was it X?" This reads as conscientious, not forgetful. It is dramatically better than silence followed by a missed deadline. 4. **For the one you're already late on, go now, with the deliverable or a date.** "This slipped on my side — here's the draft, full version Thursday" recovers most of the trust that silence destroys. Then fix the mechanism, so next week's minute-38 aside gets caught by something other than luck.
